Student Records Request

All requests for current student information must be made by the parent(s) or legal guardian(s), unless student is over the age of 18. The Parent/Guardian name must be named in our computer system. Identification will be required when information is exchanged (Driver's License, Passport, or Military ID). Please submit this form with all the requested information.

 

Your requested documentation will be provided no later than five (5) business days following the date of request (Ed Code§ 49069).
